Do stem cells need to be harvested and grown for injection? I don’t believe so. I think the body knows best. What we do at Caring Medical is in situ Prolotherapy, in other words we do in situ Stem Cell or Bone Marrow Prolotherapy. What this means is that we take out the stem cells directly from the bone marrow because that is the main center of stem cells in the body. Bone marrow cells, it appears, have the most potential to differentiate into cartilage cells compared to other stem cells. That is another reason we might use bone marrow versus other cells as far as stimulating or hopefully stimulating the repair of articular cartilage.

We do in situ Prolotherapy which means we take the bone marrow obviously cells directly out of the bone marrow, which is inside the bones, and then we inject it directly into the knee. So, that’s called in situ Prolotherapy. We don’t harvest the cells. Harvesting the cells is done outside the body. What some centers do, including the hospitals, they either take the bone marrow cells or they get stem cells from fat or bone marrow and then they harvest them, which means they grow them in a lab. Or they take cartilage cells from surgery, like arthroscopic surgery and they grow them in a lab. Because it is done outside the body, it is called in vitro. They do use whole cells. When you use whole cells it is called in vivo but, again they are doing it outside the body. I don’t know about you but, I don’t want my cells being harvested and grown all over here because I just think the body knows best. Are the cells going to be healthy? What exactly are they doing? What are they putting in there? What’s happening? Just logically, cells that are outside the body cannot be the same as the ones inside the body, you can never duplicate what happens inside the body.

The best example I can come up with is breast feeding. Everybody knows breast feeding is best. Breast feeding is in situ. In other words, the baby suckles to the mother’s breast and the mother’s body will make the right kind of milk for the baby. No matter how much medicine tries to get the right formula to feed the baby instead of breast milk, breast milk is always going to be best. So, I think it is best if you take the bone marrow cells, you put them into the joint, no harvesting. You just put them directly into the joint, then the body will do what the body was meant to do to heal that joint.

A great question that I am asked is: what is the difference between in vivo, in vitro, and in situ? In other words, we always do it in the natural environment.

Natural medicine is all about the philosophy of the body knows best. The body knows how to heal its self. The body knows specifically what cells are needed and what growth factors are needed. So, let’s just provide the body the tissues, the substances that it needs to heal. We don’t need to grow stuff outside the body. When you grow cells outside the body, when it is outside the body that is called in vitro. In situ is in its natural environment like the knee joint, in vitro is outside the body. I just want to give you an example of what in situ means. We all know that the best milk for a baby is going to be breast milk. In other words, when the baby suckles on the breast they are getting milk in its natural environment right from the mother. We are not going to take some cells out of your bone marrow then harvest them for weeks or months over here and then inject them. We are going to take your healthy bone marrow cells, then we are going to inject them into your knee, and your knee in situ, in its natural environment, will do the healing that it wants.
Bone Marrow Prolotherapy – Different Kind Of Cells
Your knee might need different cells than my knee and it is not just cartilage cells, it is immune cells. Prolotherapy is proliferative therapy so; it takes a lot of different kinds of cells to get healing. If you have a meniscus in there that also need to be repaired, you need synovial cells. Why do you need synovial cells? Well synovial cell make the joint fluid. If you just take cartilage cells, lets inject a bunch of cartilage cells in there, well there are just cartilage cells. The cartilage are only nourished by the joint fluid, and maybe your knee needs some more joint fluid so, maybe you need the replication of some synovial cells. You see the body knows best.
A patient may say to me “Dr. Hauser you don’t have any proof of that.” In one way the person is right in saying that because I am not going to autopsy them and sacrifice them and see what’s going on in their knee. But me as a doctor I can tell you what a patient tells me. They tell me “Doc, I have less stiffness.” Ok, so how do they have less stiffness? It could be that their joint has more joint fluid. When your joint is all constricted and you don’t have any joint fluid, it is hard for me to push three cc’s of fluid in there. Then when that patient comes in for their forth or fifth visit I can put 10 cc’s of Bone Marrow Prolotherapy or another Prolotherapy solution in their knee. So, obviously something happened and if you have less clicking, less grinding, less stiffness, less pain. At minimum, lets at least agree that there has been some repair in there. Do I really care if it is more joint fluid, more cartilage calls, it is more meniscus? All you really care about is you got back to your activity level and me as a doctor that I have met your expectations.
Bone Marrow Prolotherapy - Did Prolotherapy Meet Your Expectations
This is one of the questions we ask, especially when we do our studies. Did Prolotherapy meet your expectations? And the resounding answer is “yes.” In over 90% the resounding thing folks say is “yes.” If on a first visit if I think someone is kind of an iffy case I will tell them “I think you are kind of an iffy case.” If somebody is an iffy case, it is a guarded prognosis meaning the joint is so far gone that they are a candidate for joint replacement, but they want to try Prolotherapy then we have an agreement. We are going to try Prolotherapy because in some instances it really still does great work, but if the prognosis is guarded and we tried a few injections and it just didn’t do anything, let’s agree to not try anymore and the person has to get a joint replacement.
I am not against joint replacements. I am not against orthopedic surgery. Let’s try conservative care first because if we can eliminate ninety percent of orthopedic surgeries for just arthritis and other diagnosis that have to do with degeneration, degenerated meniscus, degenerated cartilage, then we should do so. We have an obligation to do so. I think the treatment that has the most promise for the future as far as eliminating orthopedic surgeries is Prolotherapy and I have tried to be one of the doctors to document that in studies that we have published. What conditions can be treated with Bone Marrow Prolotherapy? It is the same conditions that normal Prolotherapy could used for. Traditionally Prolotherapy is used for ligament and tendon injuries; any kind of ligament sprain, tendon sprain, degenerated tendon, degenerated ligament, torn tendon, torn ligament. We are excited about Prolotherapy as it relates to articular cartilage injuries. There’s folks that have degeneration of the articular cartilage which typically is called chondromalacia. There are people, especially athletes, who have articular cartilage lesions. They will get an arthroscopy and basically a part of the articular cartilage is missing. A certain part of their knee, like the articular cartilage, looks good but because they had a fall or they had a trauma, there might be like a one centimeter part of their articular cartilage missing.
What conditions can be treated with Bone Marrow Prolotherapy – Autologous Chondrocyte Implantation
Traditionally what they have done is Autologous Chondrocyte Implantation, which is ACI. In other words, they would go to a surgeon who would do an arthroscopy. He would take out some cartilage cells, then grow those cartilage cells for a certain specific amount of time. Then the athlete would have a second surgery where they would plant those cartilage cells right on their lesion. The problem is if you look at the results of that procedure, it doesn’t work all that well. What do we see at Caring Medical? I see all the failures of that type of surgery. I know there are folks that procedure has helped and I am glad that the orthopedic surgeons are out there. But I really have a strong feeling that people should try Prolotherapy first. In other words I feel that when people have a musculoskeletal injury, when you have a cartilage injury, that you should try Prolotherapy first and it should be the Prolotherapy physicians referring you to the orthopedic surgeon (if needed). Not that someone should get one, two, three, orthopedic surgeries then we (the Prolotherapy doctors) are seeing them. So, if a person is going to get a meniscus surgery, cartilage surgery, ligament, tendon, arthritis surgery, I feel that they should get an evaluation from a Prolotherapy doctor.
What conditions can be treated with Bone Marrow Prolotherapy – A Study On Labral Tears
If you look at the studies we have published, we have about a ninety percent rate of helping resolve their pain complaints with Prolotherapy and them not needing the surgery. When we first did the analysis as far as meniscus tears and meniscus injuries and whenever we do a study, we hire somebody (to help with the study.) We will hire somebody to review the charts and then call the clients. I don’t know if the results are going to be good or not good. Eventually we are going to do a study on labral tears. Almost every patient that we see with a labral tear gets Prolotherapy and it gets resolved, but I don’t know the specific numbers. So, we will hire somebody like a college student or a pre-med student to look through our charts, review the patients that came here for labral tears, and call them with a specific questionnaire. Well imagine our shock when we did this for MRI-documented meniscal tears and 27 of 28 patients basically resolved their pain complaints with Prolotherapy, and only one out of twenty eight subsequently needed surgery for a meniscal tear. I mean imagine how this would revolutionize meniscal care. If you have a meniscus tear, clearly you should try Prolotherapy prior to getting a surgical procedure.

As it relates to Bone Marrow Prolotherapy, specifically a doctor like myself would use bone marrow generally only in severe cases like severe cases of articular cartilage injury, or severe degenerative arthritis, or if someone had already failed Hackett-Hemwall Prolotherapy, or Hackett-Hemwall Prolotherapy got them fifty percent better and then it seemed like they were not getting any better. Then we might switch to Bone Marrow Prolotherapy. Let me give you an example. I am thinking of a 44 year-old guy who is like six foot three and is an client of ours. He has really significant degenerative arthritis in his right hip. He is an candidate for a hip replacement but, do you really want a 44 year-old getting a hip replacement? What is the goal of Bone Marrow Prolotherapy? it’s just like any procedure we use Prolotherapy for. It’s just the repair of the injured tissue. The reason to do Bone Marrow Prolotherapy versus other kinds of Prolotherapy would be that you feel like it is going to speed the healing compared to other forms of Prolotherapy. The results with Hackett Hemwall Prolotherapy are phenomenal so, there are going to be a subset of folks, for instance, who have really advanced degenerative arthritis, or they have a specific legion of the articular cartilage, or perhaps they have not healed normally with Hackett Hemwall Prolotherapy. How does Caring Medical obtain bone marrow and is it painful?
That’s a great question. How is bone marrow obtained? There are lots of ways to obtain bone marrow but, specifically you are piercing the periosteum, or the outside of the bone, by some method and then you are aspirating, or extracting, the bone marrow from inside the cell. Bone marrow specifically is the spongy material that is inside the bones. Typically, doctors get the bone marrow from either the pelvis, which is near the back, or the tibia bone. Specifically the tibia bone is this long bone that’s the shin bone.
Caring Medical again for the ease of getting the bone marrow we like to use the tibia. The tibia is a very thin bone compared to the back. So, the actual process of obtaining the bone marrow from the tibia is less painful, it can be done within seconds and it actually doesn’t feel much different from normal Prolotherapy and we have been very happy with this method as far as obtaining the bone marrow.

For one second you feel a little pinch when the drill goes through the periosteum, but it does not feel any different than if somebody had regular Prolotherapy. I should say that the area is anesthetized. We use procaine but can use any type of anesthetic. To answer the question, it does hurt but just for a second and for me it hurt a lot less then I thought it would, and it really was not any big deal.

Why use Bone Marrow Prolotherapy – Know What Prolotherapy Is
A lot of folks have never herd the term Prolotherapy. Just to make it clear, Prolotherapy is a treatment that stimulates the repair injured tissue such as musculoskeletal tissue like ligaments, tendons, cartilage, by inducing the proliferation of cells. So, when we talk about Prolotherapy, we are specifically talking about an injection procedure using substances that stimulates the proliferation of cells to repair tissues like ligaments, tendons, meniscus, or cartilage.
Why use Bone Marrow Prolotherapy – The Most Common Solution
There’s lots of different solution ingredients that are used in Prolotherapy. The most common solution, believe it or not, is just a concentrated amount of dextrose or sugar. That’s the Prolotherapy solution that is most commonly used in Hackett Hemwall Prolotherapy. I was Dr. Hemwall’s associate, so obviously I use Hackett Hemwall Prolotherapy. However, we can use a lot of different Prolotherapy solutions. Some of the solutions that are used in Prolotherapy are combinations of various hormones in them like testosterone or growth hormone. Why would we use those solutions? It’s because ligaments, tendons, cartilage, have receptors for hormones such as testosterone. So, if you inject testosterone or other hormones, it will stimulate those receptors and it causes the proliferation of ligament tissue, tendon tissue, cartilage tissue.
Now we are progressing to the point where now we can inject directly into ligaments, tendons, cartilage tissue cells like stem cells from bone marrow to hopefully speed up the healing process and get even better results. You have to realize that traditional Hackett Hemwall Prolotherapy already gets great results.
Why use Bone Marrow Prolotherapy – The Bone Marrow
At Caring Medical we’re starting to use actual bone marrow to hopefully even improve on the results that we have already gotten. Specifically, bone marrow had a lot of cells that stimulate healing the proliferation of cells. The most common cell that people know is stem cells. Stem cells are undifferentiated cells they are called primitive cells. So, you have this one cell, and the stem cell specifically is a self-renewing cell. It just means that it can keep replicating itself. Just one cell can form hundreds of cells of itself. But specifically, this cell can change into a muscle cell, this cell can change into a ligament cell, cartilage cell, tendon cell. So, just by one cell it can form all these other cells.

What are growth factors?Why are growth factors important?
What are growth factors? Well in regard to healing, growth factors are the directors. When we talk about Prolotherapy, again Prolotherapy is an injection technique that stimulates the repair of injured musculoskeletal tissue, like ligaments, tendons, cartilage. So, when we inject various substances into these injured areas it is actually the growth factors that come to the area or are injected into the area that actually direct the healing.
Growth Factors – Different Kinds Of Initials
So think of growth factors, like platelet derived growth factor, fibroblastic growth factor, vascular growth factor, BMP, there is all the different kinds of initials and names but, specifically what they do is they direct the healing process. When doctors use bone marrow cells for instance, in Bone Marrow Prolotherapy, stem cells, platelet rich plasma, these solutions have various growth factors in them like platelet derived growth factors. These growth factors speed up the healing process and can stimulate the repair of ligament, tendon, and hopefully cartilage tissue by the induced proliferation of cells. Do Prolotherapy Results Last – It’s Ok To Get Booster Treatments?
A common question that I am asked is does the effect of Prolotherapy last. So, in other words, you have some knee arthritis, a little bit of knee arthritis. You get some Prolotherapy and now you are pain free. Patients ask “Doc, am I going to have to get booster treatments?”
Well, typically the answer is “no” but, once the Prolotherapy healing stops in the joint, the joint will start to degenerate at the rate of the rest of your body right? Prolotherapy doesn’t eliminate the aging process. So the point is why don’t you age well? Normally, while you’re getting Prolotherapy, it is a good time to start getting healthy. You start eating right, taking some supplements, exercise. So, generally we just let folks know that you’ve just got to age well.
Do Prolotherapy Results Last – The Best Things To Do
So, if you age well, the pain typically won’t come back and you won’t need further Prolotherapy. But that’s dependent on you doing everything you need to do to age well, which is eating well, getting good sleep, taking some supplements, and regularly exercising. As a person ages, the very best exercise is exercise that has to do with range of motion. Because when you have good range of motion of your joints, it means that the body producing a lot of joint fluid.
Do Prolotherapy Results Last – It Is Need To Do Different Activities?
It is non-pounding and you can do it the rest of your life. It doesn’t preclude running, or doing all the other activities. But really, as you age, most people are going to have to cross train a little bit. So, even if they don’t know how to swim, I leaned how to swim as an adult. I would just really encourage people to learn to swim. As long as they do those actives and maintain their range of motion, they typically won’t need Prolotherapy in the future.
